The fourth edition of The Treatment of Epilepsy appears five years after the previous edition. Dr Wyllie states that "the purpose of the book is to help us all do our best to care for our patients and their families." Have the editors met this goal?

The text is well organized and referenced and has six main parts. New contributors have rewritten many of the chapters. The first section, on the pathologic substrates of epileptogenesis, contains a newly rewritten chapter on brain development and malformations, a superb presentation of an extremely complex process. The rewritten chapter on epidemiology is also excellent.

The clinical contributions are clearly presented and well supplemented by electroencephalographic images. The chapters on epileptic syndromes are clear and not overwhelming as in some of the European texts.
